First 1000 Days

The First 1,000 Days Program (F1KD) serves as a welcoming "light touch" point of contact for pregnant persons and families with children up to age 5 within Chemung County. Community Health Workers (CHWs) engage families and build trusted relationships from the very beginning. They take time to understand each family’s unique strengths and needs through supportive screening and conversation.

Based on each family’s needs, Community Health Workers (CHWs) connect them with programs within CIDS and a wide range of community resources. They provide hands-on support to navigate services such as food and nutrition assistance, stable housing, transportation, healthcare, and behavioral or mental health supports.

While the program is short-term, CHWs serve as a consistent and reliable partner—helping families take the first steps toward their goals, overcome barriers, and access the resources they need. By offering personalized guidance and support, the program reduces stress, removes obstacles, and ensures families are quickly connected with the services that can help them thrive.
